When developing the supplier diversity program for our customers, we focus on their expectations as a whole; we work to realize their goals on a community, corporate, division, department and individual basis. Realizing that ZeroChaos is also part of a larger community, we give back to the community by volunteering and donating resources time to community events. The value of the charitable contribution varies by year but our goal is to minimally average $30,000. Some of the organizations that we contribute to or donate time and resources to are: Boy Scouts of America, Diabetes Foundation and the Juvenile Diabetes Foundation, The American Cancer Society, The Ronald McDonald House, The National Minority Supplier Diversity Counsel and the Second Harvest Food Bank to name some. Additionally we have a matching donation program for our employees who are provide money to charity organizations.
- We budget annually for these donations and contributions and are able to report on our progress against the goals obtained once a year.
- Internal senior review is held once a year.
- Management reviews will be conducted quarterly in the form of a leadership forum.

Environmental protection is a management responsibility as well as the responsibility of every employee of ZeroChaos. This policy concerning environmental protection addresses aspects of ZeroChaos’ operations which can potentially impact the environment.
ZeroChaos shall seek to:
- Conduct our operations in compliance with applicable laws, regulations and standards concerning environmental protection and provide a mechanism for self-monitoring to ensure compliance and continual improvement.
- Establish corporate environmental objectives and targets relevant to the environmental impacts of our activities and a means to measure progress against these objectives. These objectives and targets shall be reviewed and revised as necessary on an annual basis.
- Continually improve environmental management policies, programs, and performance, based on the results of our periodic reviews and taking into account regulatory developments, customer needs, technical developments, scientific understanding, and community expectations.
- Minimize the environmental risks to our employees and the communities in which we operate.
- Promote employee awareness of environmental concerns, actions and responsibilities.
- Promote the adoption of environmental protection goals and practices by contractors and suppliers acting on behalf of ZeroChaos, strongly encouraging improvements in our contractors' and suppliers' practices to make them consistent with those of the Corporation.
- Conduct ZeroChaos’ activities taking into consideration the efficient use of energy and materials.
- Reduce and, where possible, eliminate waste through recycling, and handle and dispose of all waste through safe and responsible methods.
- Assess the environmental condition of property interests that are acquired by ZeroChaos and appropriately address the environmental impacts caused by these properties.
- Ensure that the ZeroChaos executive staff is fully informed about pertinent environmental issues and ZeroChaos’ Environmental Policy.

The ZeroChaos Corporate office is located in a Green-Certified (ENERGY STAR) facility in downtown Orlando, FL. The following description demonstrates the ZeroChaos commitment to environmental awareness in all facets of our operations including property location management.
The ENERGY STAR is the national symbol for energy efficiency in America. Commercial and industrial facilities account for half of all energy consumption in the U.S. at a cost of over $200 billion per year, more than any other sector of the economy. These facilities are also responsible for nearly half of U.S. greenhouse gas emissions which contribute to climate change. Since the ENERGY STAR for commercial buildings was first introduced in 1999, thousands of buildings across the country have earned the ENERGY STAR and are saving billions in energy costs. The energy performance of commercial and industrial facilities is scored on a 1-100 scale and those facilities that achieve a score of 75 or higher are eligible for the ENERGY STAR, indicating that they are among the top 25% of facilities in the country for energy performance Commercial buildings that have earned the ENERGY STAR use on average 35% less energy than typical similar buildings and generate one-third less carbon dioxide. Increasing concern about the financial and environmental risks associated with climate change is driving more organizations to strive for the ENERGY STAR for their buildings, as it is seen as a symbol of an organization that is working to reduce global warming and its impacts.

Procedures and Objectives
Commitment to paperless consumption
ZeroChaos lead our industry by being one of the first companies to go completely paperless in our operations. In 2002, ZeroChaos made the commitment to reduce our paper consumption by 90% within one year; and we achieved that goal in seven months. We have continued to operate, and will do so in the future, without paper ever since largely aided by our online ZC Web processing platform. Not only are paper time and expenses and invoices eliminated, but also paper approvals, job requisitions and resumes, and employment documentation.
Time and expense system
ZeroChaos has always worked toward a paperless system with electronic time and expense management and invoicing, and will continue to do so. The ZC Web system, which is web native from inception, will continue to eliminate the need for paper time sheets, expense reports, and invoices.
ZeroChaos also opts for the electronic storage of files and documents rather than hard copy storage. Our electronic onboarding process for both our internal W2 employees and our client supplier resources on average eliminates 7 documents that would have otherwise been printed, signed, faxed or mailed and stored. It has resulted in a 92% reduction of use of paper from on boarding our W2 employees. When you consider, the number of resources/employees that Zero Chaos on-boards both internally and on behalf of our clients, this is extremely impactful and significant.
In 2011 ZeroChaos processed 10,655 resources for our clients utilizing 31,774 electronic on boarding documents. During this period, 36% of all resources, both our W2 and supplier partner resources combined, utilized electronic enrollment documents. For 2012 our goal is to have 100% of our W2 employees utilize electronic enrollment documentation and increase our clients’ supplier partner use of this service.
Live check reduction
One additional area that we put significant effort was to minimize the number of live checks being printed. Compared to the industry aver of 70% direct deposit ZeroChaos has been highly successful in this effort. Currently, 92% of our employees receive direct deposit and their pay statements electronically thereby eliminating weekly paper statements printing and envelopes for mailing. ZeroChaos will work toward increasing this to 100% adoption. Most recently we are introducing “pay cards” as an additional paperless method of payment. We are currently working on rolling this option out across the US market and plan to expand it globally in 2013
Additionally we shifted the payment of the 4,000+ suppliers under our management to EFT, eliminating 100% of the checks being printed.
We also rolled out an online site where employees and vendors can download their pay stubs online. Our goal is to reach 100% use of an electronic form of payment to both our resources and supplier partners.
Movement activated lighting system
The ZeroChaos office building is, and will continue to be, equipped with a movement-activated lighting system. When there is no movement in an office or work area for 10 minutes, the lighting shuts off automatically. Lights turn back on when detecting new movement in the area. This power savings is realized on a day-to-day basis.
ZeroChaos implores the practice of paper recycling. While we are operating in a paperless-promoted environment, there are still instances where the business uses paper. Paper recycling bins are conveniently located throughout the office with signs posted as reminders to recycle. It is ZeroChaos goal to increase paper recycling to the greatest extent possible while still maintaining operational efficiencies.
Computers are, and will continue to be, set to enter a power save mode after a period of inactivity. Almost every computer and monitor made has power save functionality, but only a very small fraction of computer users know about it or use it. ZeroChaos enables power save on every users computer and monitor after 15 minutes. Given that all ZeroChaos computers and monitors have this capability, we are able to both save money as well as cut CO2 emissions.
ZeroChaos has always encouraged the use of ceramic coffee cups to eliminate Styrofoam and plastic, and will continue to do so. This fact is clear-cut: one reusable cup per employee per year along with associated cleaning liquid is far less expensive than providing that same employee two or three disposable cups per day. From an environmental perspective, the impact of this change is significant even on the smallest scale of one person. Complete lifecycle analysis shows that a reusable cup over its average life of 3,000 uses is responsible for 30 times less solid waste and 60 times less air pollution than any disposable alternative.
Encourage employees to bring lunch to work
When considering that millions of workers drive out at lunchtime every day, it becomes clear that massive amounts of CO2 emissions are being generated. For those who work in the office, ZeroChaos provides an eco-friendly dining area and encourages people to use it. Comfortable seating arrangements; refrigerators to store lunches; microwaves, toaster ovens, and equipment to prepare food; a dishwasher for clean-up; all contribute to a worker- and eco-friendly environment to spend a lunch hour.
ZeroChaos takes advantage of every opportunity to hold meetings in a virtual environment rather than requiring travel of participants, whether by conference call or webinar. We will continue to do so and thus save all costs associated with travel, such as fuel costs, and the effect of fuel emissions on the atmosphere.

County Criminal Records Search
This report identifies if your candidate has convictions or pending cases at the misdemeanor and/or felony level. County criminal searches can be performed in an a la carte format, or paired with address trace logic to search each county court where candidate has resided.
Statewide Criminal Records Search
This report identifies if your candidate has convictions or pending cases at the misdemeanor and/or felony level by searching available statewide records in states where an candidate has resided. Statewide criminal record searches may include state criminal repositories and criminal justice records, to the extent available, where a candidate has resided.
Federal Criminal Records Search
This report identifies if your candidate has a history of federal crime, which generally includes offenses which violate federal law, occur on federal property, or across jurisdictional lines. Federal crimes may often include “white collar” offenses. The search reveals criminal records for incidents that are prosecuted in the US District Courts, such as identity theft, Internet use violations, embezzlement or kidnapping.

This database features hundreds of millions of criminal records and active integration with over 1000 sources, including detail from all 50 States, Washington DC, Guam and Puerto Rico. This report conducts research via the National Sex Offender public website, which searches information about sex offenders for all 50 states, the District of Columbia, Puerto Rico, Guam and many US tribes.

Project managers can create their own projects, link to existing company-fed hierarchy data, attach electronic contract documents, and select existing or request new service provider enrollments. Spend-level authorization and compliance approvals are automatically routed and recorded within the online project record. Project milestones, billing amounts, and methods (fee-based, unit, time and expense) are defined within the specific project accounting model and provides the framework to monitor, control, and approve service provider billing.
Real-time monitoring of all activity
All SOW engagement activity is stored within our database and is viewable via customized dashboards, pre-established engagement reports, or by customized queries.
Service providers enroll within the system and are vetted against regulatory and customer policy requirements. They can also designate resource workers, and initiate online service invoicing. Project managers, department leads, program sponsors, etc. can view project activity and spend online via customized dashboards or through our self-service reporting engine.
Service and spend progress are monitored through the engagement dashboard screens highlighting invoicing activity, worker deployment, and approved budget “burn” progress. Change orders to extend the service dates and estimated budget amounts are initiated within the system and solicit/capture needed financial approvals.

Project closure is performed within the engagement record, allowing the customer buyer to record final service completion, close out remaining unspent amounts, and complete an independent contractor or small business performance survey.

Social Security Number Validation and Address Trace Report
This report validates the Social Security Number (SSN) using information and number assignment methodology from the Social Security Administration (SSA). Supplemented by credit bureau records, the names and addresses associated with a Social Security Number will also be reported. The address trace provides a candidate’s history of addresses based on where the candidate has established credit. For any SSN issued before June 25, 2011, the Social Security Number Validation identifies the year and state of issuance and checks the SSA Death Index to help detect anomalies. This report may supplement other screening services, such as criminal background investigations.
Verify your candidate’s identity and eligibility work in the United States by accessing the resources of the Social Security Administration and Department of Homeland Security. ZeroChaos’ electronic I-9 integration with E-Verify allows our clients to easily perform verifications and process non-confirmation letters.
Motor Vehicle Records
Check This report confirms the validity and status of a candidate’s driver’s license. Additional information including license type, endorsements, restrictions, driving violations, suspensions and revocations will also be disclosed. Our Motor Vehicle Records (MVR) report provides this information and shows driving under the influence (DUI) convictions for states that report this information in driving records.
Credit History Report
This report provides the credit history of a candidate as reported by the US national credit reporting agency and will disclose information such as creditors, account balances, negative accounts, accounts in collections and inquiries by third parties. Special terms and condition apply to the use of employment credit reports.
Sanction and Watchlist Records
OFAC Prohibited Parties Search
This report searches U.S. databases and watch lists, including Office of Foreign Asset Control (OFAC). Confirmed listings will identify if a candidate may be a known terrorist, specially designated national, narcotics trafficker or other sanctioned individual by the U.S. government. This search helps identify candidates who are not allowed to do business in the U.S. or deemed to be a threat by the federal government.
This report searches records as provided by the Department of Health and Human Services Office of the Inspector General (OIG) List of Excluded Individuals/Entities. Confirmed listings determine that a candidate is currently excluded from participation in Medicare, Medicaid and all other federal health care programs by accessing for exclusion actions taken by the OIG.
This report searches the General Services Administration’s (GSA) Excluded Parties List System. Confirmed listings determine that a candidate is currently excluded from receiving federal contracts, certain subcontracts and certain types of federal financial and non-financial assistance and benefits. The GSA search references for debarment actions taken by various federal agencies, including actions taken by the OIG.
FDA Debarment List Search
This report searches the U.S. Food and Drug Administration (FDA) Debarment List to verify if the candidate has been convicted of a felony for violating the Federal Food, Drug, and Cosmetic Act (FD&C Act). Confirmed listings indicate that a candidate has been barred or precluded from doing research or being involved with biologics, cosmetics, drugs, foods, medical devices, radiation-emitting electronic products or veterinary products.
Verification and Reference Services
By directly contacting the institution or its authorized agent, ZeroChaos will confirm a candidate’s credentials by verifying that a degree, certificate or diploma was received. We will report any variances indicating the candidate has falsified education information on their resume or job application.
Professional Credentials and License Verification
By directly contacting the issuing authority or its authorized agent, ZeroChaos will confirm a candidate’s professional license or credentials. We will report any variances indicating the candidate has falsified information, concealed disciplinary actions, or expired status.
By directly contacting the current/former employer or its authorized agent, ZeroChaos will confirm a candidate’s work history, such as company names and locations, dates of employment, positions or titles held, and eligibility for rehire. We will report any variances indicating the candidate has falsified employment information on their resume or job application.
Professional Reference Check
ZeroChaos’ professional reference check services involve our researchers contacting the candidate’s former colleagues and coworkers and conducting a brief interview using open-ended interview questions to seek out insight as to the candidate’s character and professional conduct.

Substance Abuse Screening
Test for the presence of illicit drug use with urine, oral fluid or hair testing. ZeroChaos provides scalable options including and array multi-panel lab-based tests to fit an employer’s needs. We partner with federally certified laboratories that meet the DOT standards. Results will be supported by a MROCC certified Medical Review Officer (MRO) who reviews non-negative test results and investigates for possible medical explanations and legitimate use of prescribed drugs to determine a final test result.

ZeroChaos offers an extensive network of collection sites that use eCCF technology for non-DOT regulated drug screening. Using our eCCF passport technology during the drug screening process increases efficiency, helps to ensure information is sent to the correct collection site location, and allows drug testing program managers to track drug tests through the entire process.
Rapid Drug Screening
ZeroChaos offers a scalable national solution for rapid results drug screening. Many of our in-network clinics are installed with a sophisticated appliance that uses an under-seal specimen cup & digital technology to scan the smart cup, providing an automated, objective, confidential testing process. This leading edge technology electronically captures the drug test results without the subjectivity of a clinician interpretation. Within about 15 minutes, the appliance automatically reports a negative urine drug screen back to you via our online interface. Any non-negative specimens are expedited to our centralized lab for confirmation and applicable MRO. Some limitations apply. Consult your Account Representative for details.
Test for the presence of alcohol use. This screening is typically engaged in post-accident, reasonable suspicion circumstances or to comply with DOT random testing requirements. ZeroChaos offers several alcohol test methods such as breath and oral fluid tests.

Our international reports are assembled through collaborative methodology with internal resources, government agencies and professional field researchers. Identify a candidate who may have a criminal history. The international criminal records search may be performed by searching through data sources such as local police, national law enforcement or local court files in a selected jurisdiction, to the extent possible.
International Education Verification
Identify whether a candidate has falsified education information on their resume or job application. Our international education verification confirms a candidate’s credentials by verifying a degree, certificate or diploma claim directly with an institution or its authorized agent.
International Employment Verification
This service confirms a candidate’s work history, such as company names and locations, dates, positions or titles held along with compensation, directly with former employers or their authorized agents.
